What it involves

Looking at the whole person, not just a symptom

Geriatric care assesses an older person's physical health, memory, mood and independence all together. At home you notice things that go unseen in a consulting room: how they move about, what fall risks there are or how they take their medication. With that, treatment can be adjusted and complications prevented.

What's included

  • Physical, cognitive, emotional and functional assessment
  • A full review of medication and its interactions
  • Management of chronic conditions: diabetes, high blood pressure, COPD
  • Fall prevention and advice for the home
  • Coordination with family members and carers

When to request it

  • Chronic conditions that need regular monitoring
  • After a fall or a loss of independence
  • Changes in memory, confusion or changes in mood
  • When they take a lot of medication that should be reviewed

Who it's for

  • Older people who live alone or with their family
  • Patients with several chronic conditions
  • People with reduced mobility or who are dependent on others
  • Families and carers looking for guidance

Have private health insurance? We work with insurers and intermediaries: ask during the call whether your policy covers the visit.

Step by step

What the visit is like

No waiting rooms, no travelling: it all starts with a phone call.

  1. Request a visit

Call us or fill in the form with your name, phone number and postcode. You can request it for yourself or for a family member.

  2. Phone assessment

A healthcare professional calls you, asks about the symptoms and confirms that a home visit is the right option. If it isn't, we will tell you.

  3. Assessment in their own surroundings

The doctor visits the older person at home, reviews their health and medication and shares the recommendations with the family.

Frequently asked questions

What people usually ask us

  • How is it different from a general consultation?

Geriatric care looks at the older person as a whole: physical, mental and functional health, medication and surroundings. Rather than treating an isolated symptom, it considers how everything affects their daily life.

  • Do you coordinate with the family or carer?

Yes. With the patient's consent, we keep in touch with family members and carers to provide continuous care.

  • Do you offer palliative care at home?

Yes. We have doctors trained in palliative care to support patients with advanced illnesses, and their families, at home.

  • Does the visit include a medication review?

Yes. We review all treatments, identify duplications or interactions and suggest adjustments when necessary.

Would you like an assessment for an older relative?

Call us and we will explain how we organise the first visit and the follow-up.

Is it a life-threatening emergency?

We do not handle life-threatening emergencies. For chest pain, severe difficulty breathing, loss of consciousness, signs of a stroke or heavy bleeding, call 112.
